4 1/4 by 11 piece of cardstock. Score at 10, 6, 4.

Turn it over score at 5

Designer Series Paper; 3 3/4 by 4 and 3/4 by 4

Ribbon

Small clear bag

Scotch tape

And whatever embellishments you want to put on the front
